Box 23 - Prior Authorization Number

Box Definition

Box 23 on the CMS-1500 form is intended to display the payer assigned authorization number for services.

To change the Prior Authorization Number on the claim form:

1. Navigate to the patient's Treatment Episode.
2. Select the Scheduling & Utilization tab.
3. Select Edit for the Utilization Plan for the claim's dates of service.
4. Enter the Authorization Number.
5. Select Save.
