Box 6 - Patient Relationship to Insured

Box Definition

Box 6 is where the patient's relationship to the insured is entered.

To change the patient's relationship to the insured (Box 6) on a claim form:

1. Navigate from the Patient to Intake > Insurance.
2. Select the insurance set that needs to be updated.
3. Select the Policy Holder tab.
4. Click Edit
5. Edit the Patient Relationship to Insured field.
   
   
6. Click Save.
