Box 3 - Patient's Birthdate, Sex

Box Definition:

Box 3 on the CMS-1500 is where the Patient's Birthdate and Sex are entered.

To change the Patient's Birthdate or Sex on the claim form: 

1. Navigate to the Patient's Intake Profile.

2. Select Edit.

3. Edit the Gender or Birthday field.

4. Click Save.
