Box 4 - Insured's Name

Box Definition

Box 4 on the CMS-1500 Form is where the subscriber's name is entered as shown on their insurance ID card. 

In Kipu RCM

• If the patient is the subscriber, clicking on the Patient Profile section of a treatment episode allows you to manage the patient's name. For more information on managing patient demographics, click here. 
• If the subscriber differs from the patient, the name can be updated within the Policy Holder section of the patient's profile. For more information on updating the policyholder information, click here.
