Box 4 - Insured's Name

Box Definition

Box 4 on the CMS-1500 Form is where the Insured's name is entered as shown on their insurance ID card.

To change the insured's name on the claim form:

1. Navigate to the Patient's Profile.
2. Select Edit.
3. Edit the Patient's First and Last Name.
4. Select Save.
