Box 2 - Patient's Name

Box Definition 

Box 2 on the CMS-1500 is where the patient's full name is entered as Last Name, First Name, and Middle Initial. 

In Kipu RCM

Clicking on the Patient Profile section of a treatment episode allows you to manage the patient's name. For more information on managing patient demographics, click here. 

EDI Loop and Segment

• If the client is also the subscriber: Loop 2010BA, Segment NM103 (Last), NM104 (First), and NM105 (Middle Initial).
• If the client is not the subscriber: Loop 2010CA, Segment NM103 (Last), NM104 (First), and NM105 (Middle Initial).
