Box 2 - Patient's Name

Box Definition

Box 2 on the CMS-1500 is where the patient's full name is entered as Last Name, First Name, and Middle Initial. 

In AveaOffice

To change the patient name on a claim form:

1. Navigate to Patients and click on the patient name from the list. 
2. (You can also locate the patient using Search.)
3. In the Profile tab, click the Edit button.
4. Edit the First Name, Last Name, Middle Initial, or Suffix field.
   

EDI Loop/Segment

If the client is also the subscriber: Loop 2010BA, Segment NM103 (Last), NM104 (First), and NM105 (Middle Initial).

If the client is not the subscriber: Loop 2010CA, Segment NM103 (Last), NM104 (First), and NM105 (Middle Initial).
