Box 1 - Plan Type

Box Definition

Box 1 on the CMS-1500 is where the insured's plan type is shown depending on the payer.

To change the insured's plan type on the claim form:

1. Navigate to the patient's Intake > Insurance tab.
2. Click into the insurance set primary payer for the claim.
3. Go to the Payer tab.
   
4. Click the Change button.
   
5. Select a different insurance payer.
   
   
   
6. Click Select
