Box Definition

Box 5 on the CMS-1500 is where the Patient's Address is entered.


In AveaOffice

To change the patient's address on the claim form:

1. Navigate to the Patient and go to Treatment Episodes>Select Treatment Episode>Intake > Patient Profile tab.

2. Select Edit.

3. Edit the Address, City, State, Zip Code, and Phone Number (if relevant) fields.Treatment Episode Address.png

4. Click Save.
