Box Definition

Box 17b displays the Referring Provider NPI number.

Box 17b Referring Provider NPI number is chosen during UR Planning or Standalone service creation when the Referring Rendering is set in the form. 

To add or change Box 17b Referring Provider NPI for Standalone service billing:

1. Go to Attendance
2. Select the week the treatment was delivered
3. Choose to add a new standalone service (or unlock and edit an existing standalone service)
4. Select the Referring Rendering Provider from the drop down
5. Select the Referring rendering Provider role
6. Click Save

To add or change Box 17b Referring Provider NPI for UR Plan service billing:

1. Go to the patient's Scheduling and Utilization section
2. Choose to add a new UR Plan (or edit an existing UR plan)
3. Select the Referring Rendering Provider from the drop down
4. Select the Referring rendering Provider role
5. Click Save
