Box 80 - Remarks

Box Definition

Box 80 on the UB-04 claim form displays any remarks for additional information that may be needed to process the claim.


In AveaOffice

You can add remarks in Avea in two ways: 

  1. From the Work Center > Submit claims section
  2. When creating a Claim Correction. 

Add from Work Center

Remarks can also be added for institutional claims from Work Center > Submit Claims.

  1. ​Navigate to the Work Center and open the Insurance Claims tab.mceclip0__23_.png
  2. From the Submit Claims tab, locate the claim and click the Add Remarks quick link.
  3. Enter Remarks for the claim. This field only supports 24 characters per line. mceclip1.png
  4. Click Submit.

Add During Claim Corrections

Box 80 can be updated in AveaOffice during a claim correction:

  1. Navigate to the Patient's Treatment Episode > Insurance Billing > Review Corrections.
  2. Click Check for Corrections Directly.mceclip2.png
  3. Click Submit.
  4. Click Resolve. mceclip5.png
  5. Use the Remarks (Written to Claim Form) section to enter Remarks.
  6. Select Resolve.
