Box Definition
Box 80 on the UB-04 claim form displays any remarks for additional information that may be needed to process the claim.
In AveaOffice
You can add remarks in Avea in two ways:
- From the Work Center > Submit claims section
- When creating a Claim Correction.
Add from Work Center
Remarks can also be added for institutional claims from Work Center > Submit Claims.
- Navigate to the Work Center and open the Insurance Claims tab.

From the Submit Claims tab, locate the claim and click the Add Remarks quick link.
- Enter Remarks for the claim. This field only supports 24 characters per line.
- Click Submit.
Add During Claim Corrections
Box 80 can be updated in AveaOffice during a claim correction:
- Navigate to the Patient's Treatment Episode > Insurance Billing > Review Corrections.
- Click Check for Corrections Directly.
- Click Submit.
- Click Resolve.
- Use the Remarks (Written to Claim Form) section to enter Remarks.
- Select Resolve.
