Box 64a, 64b, 64c - Document Control Number

Box Definition

Boxes 64a, 64b, and 64c on the UB-04 are where the document control numbers (DCNs) are shown. This number can also be called the Internal Control Number (ICN).

• A - Primary
• B - Secondary
• C - Tertiary

To change the document control number on the claim form:

1. Navigate to the patient's Insurance Billing > Claims tab.
2. Click the claim ID that needs the document control number changed. 
3. Go to the Workflow tab.
    
4. Click Edit.
5. Add or edit the Internal Control Number field.
   
6. Click Save.
