Box Definition 

Box 57 is where another provider ID is shown. This can sometimes be the payer-assigned provider ID.

• The payer-assigned provider ID is managed within the payer's profile under the Insurance Payers section of the Practice Admin. For more information on managing payers, click here.
• A Claim Form Rule is required to write the payer-assigned provider ID to a claim.
