Box 14 - Admission Type

Box Definition

Box 14 on the UB-04 claim form is used to indicate the type of admission. The options are as follows: 

• 1 = Emergency
• 2 = Urgent
• 3 = Elective
• 4 = Newborn
• 5 = Trauma Center
• 9 = Information Not Available

In AveaOffice

To edit box 14 on the UB-04 claim form in AveaOffice:

1. Navigate from the Patient to Intake > Admit/Discharge.
2. Select Edit.
   
3. Select the Admit Type.
4. Click Save.
