Box 14 - Admission Type

Box Definition 

Box 14 on the UB-04 claim form indicates the type of admission using the following options:

• 1 = Emergency
• 2 = Urgent
• 3 = Elective
• 4 = Newborn
• 5 = Trauma Center
• 9 = Information Not Available

• A Claim Form Rule can be created to include or omit the admission type from the claim. For additional information, click here.
