Box Definition

Box 10 on the UB-04 Claim form displays patient birth date.

In AveaOffice

To update box 10 in AveaOffice:

1. Navigate to the Patient Profile.
   
2. Select Edit.
   
3. Enter the Birthdate in format (mm/dd/yyyy).
4. Select Save.
