Box 8b - Patient Name

Box Definition

Box 8b on the UB-04 is where the patient's full name is entered as Last Name, First Name, and Middle Initial.

To change the patient name on a claim form:

1. Navigate to Patients and click on the patient name from the list. 
2. (You can also locate the patient using Search.)
3. In the Profile tab, click the Edit button.
4. Edit the First Name, Last Name, Middle Initial, or Suffix field.
