Claim Form Rule - Adding or Removing Claim ID Decimal

What claim change is needed?

Adding or removing the claim ID decimal on the UB-04 and CMS-1500 claim forms

Box Number

UB-04: Box 3a

CMS-1500: Box 26

Steps

  1. Navigate to Management Center > Claim Rules
  2. Click "Create Claim Form Rule".
  3. Select the practice.

  5. Set Claim Type to either Institutional or Professional

  6. Set Conditions for claim rule to meet.
  7. Under Available Behaviors, select "Claim ID - Decimal". mceclip1.png
  8. Click Add. 
  9. Select "Omit" or "Write to Claim"mceclip3.png
  10. Click Create.
