Box Definition

Boxes 63a, 63b, and 63c on the UB-04 are where the treatment authorization codes are shown.

A - Primary
B - Secondary
C - Tertiary

In Kipu RCM

Authorization codes are automatically included on claims with U/R-required services. An authorization plan must be in place to record the patient's U/R-required service. For more information on creating U/R plans, click here.
