Box Definition

Boxes 63a, 63b, and 63c on the UB-04 are where the treatment authorization codes are shown.

• A - Primary
• B - Secondary
• C - Tertiary

To change the authorization number on the claim form:

1. Navigate to the patient's Scheduling and Utilization > U/R tab.
2. Click Edit for the U/R plan for the dates of the claim that need to be edited.
3. Edit the Authorization Number.
    
4. Click Save.
