Box 62a, 62b, 62c - Insurance Group Number

Box Definition

Boxes 62a, 62b, and 62c on the UB-04 are where the insurance policy group number is shown.

• A - Primary
• B - Secondary
• C - Tertiary

To change the insured's ID on the claim form:

1. Navigate to the patient's Intake > Insurance tab.
2. Click into the insurance set primary, secondary, or tertiary payer for the claim.
3. Go to the Policy Holder tab. 
4. Click Edit.
5. Edit the Group Number field.
   
6. Click Save.
