Box 59a/b/c - Patient Relationship to Insured

Box Definition

Boxes 59a/b/c on the UB-04 show the patient's relationship to the insured. The letters a/b/c of the box represent the relationship code for the primary, secondary, and tertiary policyholder.  The RCM supports four codes: 

• 1: Spouse
• 18: Self
• 19: Child
• G8: Other
