Box 25 - Federal Tax ID Number

Box Definition

Box 25 is used to show the unique identifier assigned to a provider by a federal or state agency. This is either a Federal Tax ID (TIN), an Employer Identification Number (EIN), or Social Security Number (SSN). 



In AveaOffice

To change the federal tax ID number on the claim form:

1. Navigate to Practice Admin > Configuration > Insurance Billing. Practice Admin Insurance Billing.png

2. Click Edit

3. Add a Scheduled Change Set for an effective date, or edit the baseline to edit the Tax Identification Number field.

4. Click Save.
