Box 18 - Hospitalization Dates Related to Current Services

Box Definition 

Box 18 on the CMS-1500 Form is where the patient's admission dates are entered. If applicable, this is also where the discharge information that corresponds with the services on the claims is entered.

• The date of admission can be updated in the Intake section of the patient's treatment episode. For more information on updating the date of admission, click here. 
• A Claim Form Rule can be created to include or omit the admission date from the claim. For additional information, click here.
