Box 13 - Insured's or Authorized Person's Signature

Box Definition

Box 13 authorizes payment of benefits to the provider of services. This can be done by entering "Signature on File", "SOF", or by using an actual signature.

In AveaOffice

The signature on file, or SOF, is defaulted to the claim and cannot be edited in AveaOffice.

*It is important for providers to have documentation on hand of the patient's authorization for payment to be made to the provider.
