Box Definition
Box 12 on the UB-04 Claim form is used to display the admission or start of care date for the patient.
In AveaOffice
To edit the Admission/Start of Care Date on the UB-04 in AveaOffice:
1. Navigate to the Patient's Treatment Episode > Intake > Admit/Discharge tab.
2. Select "Edit".
3. Enter the updated admit date.
4. Save
