Boxes 9a, 9b, 9c, 9d, and 9e are intended to display the patient's address.

• 9a - Street Address
• 9b - City
• 9c - State
• 9d - Zip Code
• 9e - Country Code if other than USA (Not editable in AveaOffice)

In AveaOffice

To change boxes 9a-e in AveaOffice:

1. Navigate to the patient's treatment episode > Intake > Patient Profile.
   
2. Select Edit.
3. Edit fields Address, City, State, and Zip Code.
   
4. Save.
