Box 8a, 8b - Patient Identifier and Patient Name

Box Definition

Box 8a is where the insurance ID is shown as the patient identifier. 

Box 8b is where the patient name is entered as Last Name, First Name, Middle Initial.

In AveaOffice

To change the subscriber ID (Box 8a) on a claim form:

1. Navigate from the patient to Intake > Insurance.
2. Select the insurance set with the effective date that covers the claim's date of service.
3. Go to the Policy Holder tab.
4. Click Edit.
5. Edit the Insurance ID field.
   
6. Click Save.

To change the patient name (Box 8b) on a claim form:

1. Navigate to Patients and click on the patient name from the list. 
2. (You can also locate the patient using Search.)
3. In the Profile tab, click the Edit button.
4. Edit the First Name, Last Name, Middle Initial, or Suffix field.
