Box Definition

Box 9 is where the other insured's name is displayed, i.e. secondary insurance

In AveaOffice

To change the other insured's name on a claim form:

1. Navigate to the Patient>Treatment Episodes>[Select Treatment Episode]>Intake > Insurance tab.

2. Click into the insurance set (depending upon primary, secondary or tertiary).

3.  Click on Policy Holder tab>Edit

4. Make necessary changes to the insured's name.

5. Click Save.

• Loop 2330A, Segment NM103 (Last), NM104 (First), NM105 (Middle Initial).
